Create a .pdf from a form and save as .pdf

I have a customer running a chiropractic business who wants to be able to give his customers a tablet, with which they can then sit down and fill out the usual form for his records of them.... but he wants it to be in .PDF when they are finished.

So basically I need the customer to sit down, fill out the form he supplies, convert it to .PDF and either email it or save it to his small windows 7 based file server !

Any ideas would be fantastic !! Cheers.

I just downloaded an Android native PDF converter, which may be of some use, if you search the market you should find it, at the moment I can only remember the file name, itop.apk.
